Please review very important information regarding operations during the COVID-19 outbreak. You can get information on specific clinics and departments below.
Entry points for patients is the front door.
Designated parking lot for patients is in front of the clinic
Designated parking lot for staff and entry point is the front door.
Patients will be screened upon entering the building and may be asked to visit the COVID tent for specific COVID screening rather than proceeding to their appointment.
The COVID tent (GREEN BULDING) is located outside and to the side of the clinic. COVID related questions can be referred to our COVID hotline at 314-566-5336/+3265325336 during duty hour and +32476760327 for after hours.

For assistance or questions with any physical to include Periodic Health Assessments (PHAs) please contact Physical Exams at 314-566-5327 on Mon, Tue, Wed, and Fri. Physical Exams continues to be closed on Thursdays.

The Primary Care Clinic will continue to see:
- Well Baby Checks (WBC) for anyone under two years old
- Provider requested follow ups (patient will present with SHAPE HCF CHECK OUT SHEET marked appropriately when seen)
- EFMP for upcoming PCS/move
- Acute symptoms
